The Gathering Place supports adults with physical and/or cognitive impairments in staying active within their communities. We aim to maximize participants’ health, independence, and overall functioning. We create a safe and supportive environment where individuals can access a variety of professional health, social, and therapeutic services. Additionally, these services provide respite, support, and education for family members and caregivers.

The Gathering Place offers social, medical/health, and specialized programs to meet the needs of our rural community. Under the oversight of a registered nurse, there is one direct care worker for every six participants, facilitating individualized, person-centered care and enabling staff to care for increasingly complex needs.
Before a participant enters our program, our registered nurse will perform a health assessment so we can create a participant centered Plan of Service that best meets his/her/their needs and capabilities.
From the assessment, we develop a personalized Plan of Service which may include the following:

With the help of families and staff, participants choose from a broad range of wellness-focused activities that reflect individual interests, talents and needs. We firmly believe learning is as imperative to life as breathing. The Gathering Place offers educational opportunities to encourage participants to try new experiences and learn new things through participation and socialization. We have in house educational programs and opportunities as well as guest speakers who share their knowledge, passion and skills.
Learning is a respectful process where older adult learners and adults with disabilities can always succeed. When diverse learning processes are offered participants know they are being treated with dignity and respect, creating a culture of engagement.
Whether a family member, friend or professional caregiver, living with and caring for someone with Alzheimer’s or other forms of memory loss can be daunting. The Gathering Place recognizes every nuance of dementia and all that it brings to the individual as well as the caregiving family. Our staff are trained in the care of those with memory loss; watching for and anticipating the subtleties.

The Gathering Place offers a light breakfast, a healthy lunch and an afternoon snack each day. TGP staff endeavors to ensure all meals are tasty and meet USDA guidelines for older adults. Individual dietary needs and restrictions are carefully followed and we offer assistance to participants with feeding challenges. Hydration is closely monitored throughout the day.
A healthy diet packed with vital nutrients can help ward off potential health problems like, heart problems, diabetes, high blood pressure and high cholesterol. Nutritious foods will help you maintain a healthy weight and can work wonders for your energy level.
The Gathering Place coordinates transportation to and from our Centers and to and from medical appointments on days of attendance. Drivers will provide, if needed, physical assistance on and off the vehicle as well as getting to and from your front door.
To accommodate transportation needs, a 48-hour notice is required. To qualify for these services you must be a resident of Vermont and meet eligibility.
